中医脉诊

 

看中医,医生一般都要把脉。大家都知道这就是中医的脉诊。有经验的医生靠脉诊就基本上能知道你身体的阴阳虚实状况和病变的大概部位。历史上不乏这样的奇人能人。但一般还是建议将望诊,问诊和舌诊结合起来分析综合再作最后诊断。毕竟人命关天,谨慎为好。

脉诊对于注重八纲五行脏腑卫气营血三焦辨证的学院派医生来说是必须顾及的,而对于注重六经阴阳药证辨证的医生,脉诊的精细程度要求就不那么高了;而对于空间医学 (即小方疗法) 体系,脉诊似乎不是必须的。所以,如果你看中医,如果你的医生把脉很快,时间很短,也许他属于不同的中医体系。

脉诊常用的部位是手腕内侧的桡动脉。左手的这个部位可以反映心,肝和左肾脏的功能,而右手的这个部位可以反映肺脏,脾脏,和右肾脏的功能。医生的手指按压的力度不同,得到的信息也不同。轻轻按压时手指下的感觉属于轻取,稍用力后感觉到的脉搏形状属于中取,而重按后手指下的感觉就称为重取。如果轻取就能感觉到脉搏搏斗,说明病变在体表;中取时才能感觉到的脉搏感觉,说明病变部位在中;重按时才能感觉到脉搏博动,说明病变深。这种一般的说法。医生一般用自己的食指,中指和无名指同时把脉,每一个部位 (每个指头下) 都有浅中沉三个层次。各自代表的信息是不同的。

由于脉搏的深浅,形状等感觉和季节,气候,身体外部和内部的温度,情绪,性别,年龄等等因素有关,所以脉诊时医生必须排除这些因素,辨别哪些感觉属于反映病情的真实脉搏,而哪些仅属于自然的因素。比如,脉搏在春天比较柔弱轻虚而滑,中医称为弦脉;在夏天来盛去盛,叫钩脉;在秋天比较轻虚以浮,来急去散,叫浮脉,而在冬天比较沉,叫作营脉。你看,脉搏的形状是否和自然界的整体状态息息相关呢?在冬天摸到比较沉的脉搏就需要鉴别是否是由于季节的影响而不是病理的表现。

脉搏的形状和人体的生理病理状况密切相关,所以病情变化的过程中脉搏的感觉也会变化。脉搏的变化就成为一个很好的指示病情的指标。

对同一个病人的脉搏,不同的医生很有可能有不同的感觉和诊断。这是临床常见的现象。除了常见的浮脉,紧脉,沉脉,细脉,弦脉,长脉,短脉,数脉,迟脉,洪脉,滑脉,涩脉等比较好理解和靠书本描述也可以掌握和区别,许多其它的脉象,比如芤脉,革脉,濡脉,劳脉,伏脉,散脉,弹石脉,输脉,屋漏脉等,相对少见,也必须靠有经验的医生指点才不至于和常见的脉象相混淆。这是另一个最明显的例子,说明现行学院式中医教育模式很难培养高质量中医医生。随着本来就已经很少的中医名医的过世,中医的断代现象已经危及到了中医的传承。大批中医学院毕业生进入社会并不等于中医力量的壮大。

脉诊可以在桡动脉 (手腕内侧)上进行,也可以在颈动脉 (脖子两侧的动脉) 上进行,也可以在足背动脉上进行。但意义不完全一样。这里不多介绍。
