2d. Six Jing diagnosis –Taiyin stage


诊断太阴病的依据是腹满而吐,食不下,时腹自痛, 自利不渴. 本质为里虚寒证。以腹胀,吐,和泄泻为特点。主治方剂是理中汤。太阴病不可用下法,若下之,必胸下结硬, 自利益甚。脉浮者可用汗法(桂枝汤)祛寒外出。

If a patient feels bloating in belly, has vomit, cannot eat, has pain the belly from time to time, has diarrhea but no thirsty, we will say that his/her condition belongs to Taiyin stage of the Shanghan disease. The nature of the Taiyin disease is inside weakness and cold. It is characterized as belly bloating, vomit and diarrhea. The main herbal formula used for the treatment is Li Zhong Tang. In the Taiyin disease, colon-cleansing therapy should not be used. If it is used, there will be entangle-hard feeling in the upper stomach and the diarrhea becomes worse. If the pulse feels floating, sweat therapy (by the use of Guizhi Tang) can still be used to expel Cold out of the body.


Taiyang, Yangming and Shaoyang stages belong to Yang stage (Yang conditions or Yang syndromes). They are mostly treated by the use of sweat therapy, colon-cleansing therapy, or Fire-cleansing therapy. The Taiyin stage here however, as well as the Shaoyin stage and Jueyin stage (will be discussed later) belong to Yin stages (Yin conditions or Yin syndromes). They need warm-up therapy. The warming herbs in the warming-up therapy commonly use herbs, such as Fuzi, Ganjiang, Wuzhuyu or Rougui. If there are Hot-Cold mixture syndromes, the warming herbs and cooling herbs (to clear the Fire) need to be used together. The cooling herbs commonly used are Huanglian, Huangqin or Zhimu, etc.


If the vomit is too severe, so much so that there will be vomit once the food comes into mouth, use Ganjiang Huangqin Huanglian Tang for the treatment.


Distinguishing diagnosis:

(1). 阳明中寒腑证:也有腹胀满和腹泻,但太阴病者能进食,进食不加重病情。而阳明中寒腑证者不能食。一吃东西就觉得腹胀,头眩,微烦,手足出汗,小便不利及腹泻。阳明中风腑证者腹胀满,但大便难,而且口渴,渴则喜冷饮且饮水多。

(1). The organ phase of Cold-attack Yangming stage: the patient also has bloating belly and diarrhea. In the Taiyin stage, the patient can still eat. Eating does not make the condition worse. While in the Yangming Cold-attack stage, the patient cannot eat. Eating makes worse the stomach bloating, head dizzy, slight irritated, sweat in hands and feet, difficulty in urination, and diarrhea. In the Yangming Wind-attack, the patient also feels bloating in stomach, but has difficulty in bowel movement, and thirsty with desire to drink lot of cold water.   

(2). 少阴病:脉微细,脉沉, 但欲寐. 腹胀轻而腹泻为主,大便多含未消化食物(食谷不化)。腹泻而口渴。渴而饮水每次量少且稍凉而非冰水。

(2). Shaoyin stage: The pulse is tiny and deep. The patient has desire to lie down to have nap. The belly bloating is slight but more diarrhea. The stool contains more non-digested food. The diarrhea is with thirsty, but every time drink small amount of water, which is only slight cold, not as ice cold.  

(3). 厥阴病:厥阴之为病,手足厥寒,脉细欲绝; 或下利清谷,里寒外热,汗出而厥.’”显然,厥阴病者有厥症,而太阴病者手足温,无厥症。另外,厥阴病有寒热错杂,而太阴病只里寒不夹热。

(3). Jueyin stage: The patient has diarrhea, feels thirsty and has cold hands and feet, (see below). In the Taiyin stage, the patient has diarrhea but the hands and feet are warm, not cold. In addition, the Jueyin stage has cold and hot mixture syndrome, while in the Taiyin stage, there is only cold syndrome.
