Chapter 7. Ganjiang series (Dried ginger)

Ganjiang is the dried ginger. It works to warm up the body, to deplete extra water or phlegm by warm “evaporation”, whereas other herbs, such as Fuling, Zhuling, and Zexie work to deplete the extra water by increase the urine.  It belongs to the same group as herb Fuzi in warming up the body so they usually are used together.

It should be pointed out that the Ganjiang here has different healing effect as its fresh form, the fresh ginger. This is a very important concept in the TCM that the transform of a herb from fresh one into dried one, the healing effect might be changed. The fresh ginger works to stop nausea: to reduce the reverse pushing-up of the Weiqi from stomach.

 

(1). 干姜附子汤

Ganjiang Fuzi Tang

乾姜一两, 附子(去皮,生用,破八片)一枚.

Ganjiang 15 gram, Fuzi (remove skin, cut into eight pieces) 30 gram.

右二味,以水三升,煮取一升,去滓,顿服。

Add the two herbs and 750 ml water into a herbal pot. Bring to boil and keep in mild boiling until there is 250 ml liquid left. Drink the herb tea all once.

14 下之後,复发汗,昼日烦躁不得眠,夜而安静,不呕不渴,无表证,脉沉微,身无大热者,乾姜附子汤主之。

If after bowel-cleansing therapy followed by a sweat therapy again, if the person feels annoyed emotionally and physically and hard to fall in sleep during the daytime, but quiet and no problem to sleep at night, no nausea, no thirsty, no hot/fever, no Taiyang Biao condition[1], but his pulse is deep and weak, herb Ganjiang Fuzi Tang should be used mostly.[2]

 

(2). 甘草乾姜汤

Gancao Ganjiang Tang

甘草(炙)四两, 乾姜(炮)二两

Gancao (processed) 60 gram, Ganjiang (processed) 30 gram.

右二味,以水三升,煮取一升五合,去滓,分温再服。

Add the herbs and 750 ml water into a herbal pot. Bring to boil and keep in mild boiling until there is about 300 ml herbal tea left. Collect the tea, separate it into several parts. Drink one part of the tea each time, repeat every 2 to 3 hours.

05  伤寒脉浮,自汗出,小便数,心烦,微恶寒,脚挛急, (当与桂枝增桂加附子汤,以温经止汗) 反与桂枝汤,欲攻其表,此误也,得之便厥。咽中乾,烦燥吐逆者,作甘草乾姜汤与之,以复其阳。若厥愈足温者,更作芍药甘草汤与之,其脚即伸;若胃气不和,谵语者,少与调胃承气汤;若重发汗,复加烧针者,四逆汤主之。

With Shanghan disease, the pulse is floating and the person has sweat and frequent urine, annoyed/impatience, slight chilly, spasm in feet[3]. (It should use Guizhi jia Guizhi jia Fuzi Tang to warm up the body and stop the sweat). If Guizhi Tang aloe is given to deplete the Xieqi from the body surface, it is a wrong treatment. It will cause reverse Jue condition (cold hands and feet). If the person feels dry in mouth, annoyed/impatience, vomiting, give Gancao Ganjiang Tang to restore his Yangqi. If his hands and feet return to warm, give Shaoyao Gancao Tang to release feet spasm. If the person has non-harmonized stomach, has Chattering syndrome, give Tiaowei Chengqi Tang. If he was given repeated sweat therapy and warm-acupuncture, use Sini Tang.

06  问曰:证象阳旦,按法治之而增剧,厥逆,咽中乾,两胫拘急而谵语。师言夜半手足当温,两脚当伸。後如师言,何以如此?答曰:寸口脉浮而大,浮为风,大为虚,风则生微热,虚则两胫挛,病形象桂枝,因加附子叁其间,增桂令汗出,附子温经,亡阳故也。厥逆,咽中乾,烦躁,阳明内结,谵语烦乱,更饮甘草乾姜汤,夜半阳气还,两足当热,胫尚微拘急,重与芍药甘草汤,尔乃胫伸,以承气汤微溏,则止其谵语,故知病可愈。

The person’s condition is similar to Yangdan (Guizhi Tang) condition. However his condition becomes worse when the Guizhi Tang is used. He feels Jue condition, dry in mouth, spasm in legs and has Chattering syndrome. The Master said the hands and feet will be warm at night and the leg will be easy to move again. Later it happens exactly as the Master predicted. How come? Answer: The pulse is floating and big in the Chun position, floating means Wind and big means weak. The Wind causes slight hot and the weak causes spasm in legs. Since the overall condition is similar to Yangdan condition, herb Fuzi is added in the formula to improve the sweat ability of the Guizhi Tang. The use of the Fuzi is due to the Yang depletion condition here. The condition of Jue Condition, dry mouth, annoyed/impatience suggest Yangming stage with Xieqi condensed. For Chattering syndrome and annoyed, use the Gancao Ganjiang Tang to improve the Yangqi restore. At middle night, the Yangqi comes back and feet should be warm. Since the feet is slight spasm, the Shaoyao Gancao Tang is used. Finally use Chengqi Tang to clear the Xieqi in the Yangming to stop Chattering syndrome, so it is predicted it will be recovered.

肺痿吐涎沫而不咳者,其人不渴,必遗尿,小便数。所以然者,以上虚不能制下故也。此为肺中冷,必眩多涎唾,甘草干姜汤以温之;若服汤已渴者,属消渴。

If the person spit lots of saliva but no cough, no thirsty, he must have urine leak, and frequent urine. This is due to the weakness in the Upper Jiao Cavity that cannot control the Lower Jiao Cavity. The condition is the Cold in the chest.[4] The person feels dizzy and spits lots of saliva or mouth water. Use Gancao Ganjiang Tang. After the drinking, it the person feels thirsty, the condition belongs to Xiaoke.[5] 

 

(3). 理中汤方

Lizhong Tang

人参 白术 甘草(炙) 乾姜各三两

Renshen 45 gram, Baishu 45 gram, Gancao (processed) 45 gram, Ganjiang 45 gram.

右四味,捣筛,蜜和为丸,如鸡子黄许大,以沸汤数合,和一丸,研碎温服之,日三、四,夜二服。腹中未热,益至三、四丸,然不及汤。汤法以四物,依两数切,用水八升,煮取三升,去滓,温服一升,日三服。

Prepare the herbs into pills to use. Or, add the herbs and 2000 ml water into a herbal pot. Bring to boil then keep in mild boiling until the liquid volume in it is about 750 ml. Collect the herbal tea. Drink one third each time, three times a day. 

加减法:若脐上筑者,肾气动也,去术加桂四两。吐多者,去术加生姜三两。下多者,还用术;悸者,加茯苓二两。渴欲得水者,加术,足前成四两半。腹中痛者,加人参,足前成四两半。寒者,加乾姜,足前成四两半。腹满者,去术加附子一枚。服汤後,如食顷,饮热粥一升许,微自温,勿发揭衣被。

Modifications: if the person has jumping feeling in the upper stomach (or above the naval), it is the movement of Kidney Qi, remove Baishu, but add Guizhi 60 gram. If with vomiting, remove Baishu, add Fresh ginger 45 gram; if with frequent diarrhea, add little bit more Baishu; if with palpitation, add Fuling 30 gram; if with thirsty and willing to drink water, add additional Baishu 23 gram; if with stomach pain, add additional Renshen 23 gram; if with Cold inside, add Ganjiang 23 gram; if with fullness in the Stomach, remover Baishu, add Fuzi 45 gram. After drinking the tea, eat rise soup about 250 ml. Cover the body and lie down for one hour to let the body feel warm.

03 自利不渴者,属太阴,以其藏有寒故也,当温之,宜服四逆辈。

If the person has self-diarrhea[6] but no thirsty, this condition belongs to Taiyin[7], since there is Cold inside. It should be warmed, by using Sini Groups.[8]

47 伤寒服汤药,下利不止,心下痞硬,服泻心汤已,复以他药下之,利不止,医以理中与之,利益甚;理中者,理中焦,此利在下焦,赤石脂禹馀粮汤主之,复利不止者,当利其小便。

After a treatment, the person has continuous diarrhea and Fullness syndrome in the stomach. Xiexing Tang is the right choice. If a bowel-cleansing therapy is given again, the diarrhea continues. Doctor gives Lizhong Tang, the diarrhea becomes worse. It is wrong. In this case, herb Chishizhi Yuyuliang Tang[9] should be the main herbal formula, since the disease is now in the Lower Jiao cavity (the lower stomach area). The herbal Lizhong Tang however works in the Middle Jiao cavity of the body (the middle part of the body). [10] For person with continuous diarrhea, urine therapy should be used. [11]

04 大病差後,喜唾,久不了了,胸上有寒,当以丸药温之,宜理中丸。

After recovery from Shanghan disease, the person has frequent spit. This is because he has Cold in the chest. Use Lizhong Wan. 

04  下利後,当便硬,硬则能食者愈。今反不能食,到後经中,颇能食,复过一经能食,过之一日当愈,不愈者,不属阳明也。(理中、脾约,择而用之可矣).

If after diarrhea, the stool turns hard, and the person starts to have appetite to eat, the disease is to recover. If the person cannot eat, it suggests that the Weiqi has not recovered yet. It has to wait for thirteen days. On the thirteenth day, if the person has good appetite, it still indicates a recover. Otherwise, it suggests the Yangming stage is not due to the hard stool, but to the weakness and cold abdomen condition, or to the weak and dryness in the abdomen after vomiting and diarrhea. This is a condition in which the treatment becomes mandatory. Choose Lizhong Tang, or Maziren Wan

05  霍乱,头痛发热,身疼痛,热多欲饮水者,五苓散主之,寒多不用水者,理中丸主之。

In Huoluan disease, if the person has more fever and willing to drink more water, use Wuling San. If he has more cold and has no strong willing to drink, use Lizhong Tang. [12]

胸痹,心中痞气,气结在胸,胸满,下逆抢心,枳实薤白桂枝汤主之,人参汤亦主之。

For person with the Xiongbi disease, if he feels fullness in the upper abdomen and in the chest, feels as there is air pushing to the heart, use Zhishi Xiebai Guizhi Tang, or use Renshen Tang.

 

(4). 桂枝人参汤方

Guizhi Renshen Tang

桂枝  四两, 甘草(炙)四两, 白术三两,  人参三两,  乾姜三两.

Guizhi 60 gram, Gancao (proceeded) 60 gram, Baishu (45 gram), Renshen 45 gram, Ganjiang 45 gram.

右五味,以水九升,先煮四味,取五升,内桂更煮,取三升,去滓,温服一升,日再服,夜一服。(本方为理中汤加桂枝)

Add the Gancao, Baishu, Renshen and Ganjiang and 2250 ml water into herbal pot. Bring to boil until the liquid volume down to about 1250 ml. Add the Guizhi in and continue to cook until the total liquid volume in the pot is about 750 ml. Collect the liquid (herbal tea). Drink one third of the tea. Drink another one third during the day and the last one third at night.

34    太阳病,外证未除,而数下之,遂协热而利;利下不止,心下痞硬,表 不解者,桂枝人参汤主之。

In Taiyang stage, the invaded disease remains but a bowel-cleansing therapy has been used repeatedly for several times, the diarrhea will be hot-diarrhea; if the diarrhea continuous, the person will feel fullness in stomach. If it is still in Taiyang stage, Guizhi Renshen Tang should be used mainly[13].

 

(5).  甘草干姜茯苓白术汤方

Gancao Ganjiang Lingshu Tang


甘草 
二两,白术  二两,干姜  四两,茯苓  四两

Gancao 30 gram, Baishu 30 gram, Ganjiang 60 gram, Fuling 60 gram.


右四味,以水五升,煮取三升,分温三服,腰中即温。

Add the herbs and 1250 ml water into a herbal pot. Bring to boil and keep in mild boiling until there is about 750 ml herbal tea left. Collect the herbal tea. Drink one third of the tea each time, three times a day.

肾着之病,其人身体重,腰中冷,如坐水中,形如水状,反不渴,小便自利,饮食如故,病属下焦,身劳汗出,衣里冷湿,久久得之,腰以下冷痛,腹重如带五千钱,甘草干姜苓术汤主之。

With Wet-invasion in the Kidney system, the person feels heavy in the body, cold in the lower back as sit in water. The body looks puffy and swelling, however do not feel thirsty. He has easy urine and normal appetite. The disease belongs to the Lower Jiao Cavity (the Liver and Kidney systems), which is caused when the person has sweat on the physical activity, the clothes is cold and wet for long time again and again. He feels cold and heavy from the lower back down to the leg as heavy as having a stone in the lower back. In this case, use Gancao Ganjiang Lingshu Tang.

 

 (6). 大建中汤

Da Jianzhong Tang


蜀椒(去汗)二合,干姜  四两,人参  二两

Shujiao 10 gram, Ganjiang 60 gram, Renshen 30 gram.


右三味,以水四升,煮取二升,去滓,内胶饴一升,微火煎取一升半,分温再服,如一爨顷,可饮粥二升,后更服,当一日食糜温覆之。

Add the Shujiao and Renshen and 1000 ml water into a herbal pot. Bring to boil and keep in mild boiling until there is about 500 ml herbal tea left. Remove the residue, add the Jiaoyi 250 ml, keep in mild boiling until the total volume is about 300 ml. Drink one third of the tea when the tea is warm. After about 30 min, eat rise soup 500 ml. After about 3 to 4 hours, drink the second one third of the tea. Always eat the rice soup and cover the body to warm.

心胸中大寒痛,呕不能饮食,腹中寒,上冲,皮起出见有头足,上下痛不可触近,大建中汤主之。

When the person has strong Cold in the chest, he feels nausea and hard to eat, feels cold in abdomen, feels air pushing up feeling. In the abdomen, it can be seen a rise of the skin, as seems has a head and feet there. Due to the pain in the abdomen, the person refuses to allow the touch on his abdomen. In this case, use Da Jianzhong Tang.

 

(7). 干姜人参半夏丸方

Ganjiang Renshen Banxia Wan


干姜
人参各一两,半夏 二两

Ganjiang 15 gram, Renshen 15 gram, Banxia 30 gram.

右三味,末之,以生姜汁糊为丸,加梧子大,饮服十丸,日三服。

Prepare the herbs into pills with fresh ginger juice. The pill is 0.5 cm in diameter. Drink 10 pills each time, three times a day.

妊娠,呕吐不止,干姜人参半夏丸主之。

If a woman in pregnancy has continuous nausea and vomiting, use Ganjiang Renshen Banxia Wan.[14]

 

(8). 乾姜黄连黄芩人参汤方

Ganjiang Huanglian Huangqin Renshen Tang is to be used.

乾姜 黄连 黄芩 人参各三两

Ganjiang 45 gram, Huanglian 45 gram, Huangqin 45 gram, Renshen 45 gram.

右四味,以水六升,煮取二升,去滓,分温再服。

Add the herbs and 1500 ml water into a herbal pot. Bring to boil then keep in mild boiling until there is about 500 ml liquid left. Collect the herbal tea, separate it into several parts to drink during the day.

04  伤寒,本自寒格[15],医复吐下之,寒格,更逆吐下,若食入口即吐,乾姜黄连黄芩人参汤主之。

If in Cold-invasion Taiyin stage, the person has Cold-block syndrome inside, but he is given a vomiting therapy or a bowel-cleansing therapy again. This results in the worse of the Cold-block syndrome, in which people has vomiting right after eating, has more vomiting and diarrhea. Ganjiang Huanglian Huangqin Renshen Tang is to be used.

 

(9). 厚朴生姜半夏甘草人参汤方

Houpo Shengjiang Banxia Gancao Renshen Tang

厚朴(炙,去皮)半斤, 生姜(洗)半斤, 半夏(洗)半升, 甘草(炙)二两, 人参 一两

Houpo (remove skin, processed) 60 gram, Fresh ginger 60 gram, Banxia (washed) 62 gram, Gancao (processed) 30 gram, Renshen 15 gram.

右五味,以水一斗,煮取三升,去滓,温服一升,日三服。

Add the herbs and 2500 ml water in a herbal pot. Bring to boil and keep in mild boiling until the liquid in the pot is about 750 ml. Collect the herbal tea. Drink one third of the tea each time, three times a day.

08 发汗後,腹胀满者,厚朴生姜半夏甘草人参汤主之。

After a sweat therapy, if the person has fullness in the Stomach, use Houpo Shengjiang Banxia Gancao Renshen Tang. [16]



[1] Biao condition means chilly, fever, headache, body pain, sweat, etc.

[2] The bowel-cleansing and sweat therapy has caused depletion of Yin of the body, making the condition deep to the Shaoyin stage. No enough Yin, so the Yang is floating. During the daytime, in which the Yang Qi is more whereas the Yin is less, so the person feels more annoyed. At night, there is relatively more Yin from the nature (the night belongs to Yin), so to balance the shortage of the Yin in the body to make the Yang combining to the Yin (to have a rest), so the person is quiet. However, some doctors believe the annoyed here is due to the depletion of the body Yang Qi, not the Yinqi.

 

Note, basically in the Taiyang stage, the person feel worse in the night, but better during the day time. Here it says the condition seems worse during the daytime – a different clinic picture. Try to make a summary, is this condition easy to been seen in most people with anxiety? They feel annoyed and their pulse is deep and weak (only a doctor know this) but no any other problem to complain?

[3] Chilly and feet spasm suggest the Cold Xieqi is invading to the lower part of the body.

[4] If the person has dry cough, no saliva spiting, it may belong to dry cough. If he has cough and spits saliva, it belongs to the Fire Feiwei. If only spit saliva, no cough, it belongs to Cold in the Lung.

[5] Xiaoke: a clinic condition in which the patient feels very thirsty, has very strong hungry feeling, or has too frequent and too much urine. Type I diabetes can have such phenomenon, but it is not the only disease that can have such Xiaoke disease.  After drinking the Gancao Ganjiang Tang, the person feels thirsty, it suggests that the condition does not belong to the Cold lung, but to Fire in stomach (Xiaoke).

[6] The diarrhea here means the diarrhea that is not caused by a wrong sweat, vomiting or bowel-cleansing therapy. It comes by itself, suggesting that the body has Cold inside.

[7] If a diarrhea with thirsty, it indicates a Fire in the inside. Now, the diarrhea is without thirsty. So it is a mark of Cold inside of the Taiyin stage.

[8] Sini Goups: several herbal formula work similarly to warm the Yin conditions, such Sini Tang, Lizhong Tang, Jianzhong Tang, Baitong Tang, Tongmai Sini Tang, etc. It is easy to distinguish the Taiyin diarrhea with the Yangming diarrhea for the diarrhea with hot and thirsty in the Yangming. There is diarrhea also in Shaoyin and Jueyin stage, both has thirsty. Thirsty in Shaoyin is mild in intensity, it is very extensive in Jueyin stage. The thirsty in Jueyin is released when the person has hot feeling but worse when the hands and feet feels cold (reverse cold).

[9] This herb formula works to contract the intestine, to reduce its movement, to increase water absorption, so to stop the diarrhea.

[10] The chest and the stomach are regarded as a whole Jiao cavity. The Heart and Lung are in the Upper Jiao cavity; the Spleen and Stomach are in the Middle Jiao cavity, and the Liver and Kidney are in the Lower Jiao cavity.

[11] It is a common concept in TCM: if the water goes to the colon to cause diarrhea, the person tends to have hard in urine; if he has too much urine, he also tends to have constipation. Only in very severe conditions, the person has both too much urine and diarrhea (loss control in urine and bowel movement); or stop of both urine and bowel movement.

[12] The treatment of Huoluan depends on the Fire or Cold condition of the person. If he has more Fire, use Wuling San. If he has more Cold in the body, use Lizhong Wan.

[13] If in this situation, the pulse is not weak, Gancao Xiexing Tang can be considered.

[14] This is a condition in which the person has Cold in stomach before the pregnancy. Now the Cold in the stomach prevent the development of the fetus.

[15] In original version, it is bowel-cleansing therapy, not the “Cold-block syndrome” .

[16] Stomach fullness after a sweat therapy suggests that the condition has turned into the Taiyin Stage.  It suggests that the person may be weak in the Spleen system usually, before the treatment.
