第八章.治心病方

Chapter 8. Herbs for heart disease

 

1.定心汤

1. Dingxin Tang (Ding means calm here in Chinese; Xin: heart; Tang; herbal tea)

治心虚怔忡。
This formula works for palpitation.

龙眼肉(一两) 酸枣仁(五钱,炒捣) 萸肉(五钱,去净核) 柏子仁(四钱,炒捣) 龙骨(四钱,捣细) 牡蛎(四钱,捣细) 生明乳香(一钱) 生明没药(一钱)
 Longyan Rou 30 gram, Suanzao Ren 15 gram (fry and grind), Shanyurou 15 gram (remove kernel), Boziren 12 gram (fry and grind), Longgu 12 gram (grind), Muli 12 gram, Rouxiang 3 gram, Moyao 3 gram.

心因热怔忡者,酌加生地数钱,若脉沉迟无力者,其怔忡多因胸中大气下陷,详观拙拟升陷汤后跋语及诸案自明治法。
The palpitation is due to Heart Hot, add Shengdi several grams. If the pulse was deep, late and weak, it is mostly due to the sink of Big Qi in the chest. Check the chapter for Shengxian Tang to choose corresponding herbs.

《内经》谓心藏神,神既以心为舍字,即以心中之气血为保护,有时心中气血亏损,失其保护之职,心中神明遂觉不能自主而怔忡之疾作焉。故方中用龙眼肉以补心血,枣仁、柏仁以补心气,更用龙骨入肝以安魂,牡蛎入肺以定魄。魂魄者心神之左辅右弼也,且二药与萸肉并用,大能收敛心气之耗散,并三焦之气化亦可因之团聚。特是心以行血为用,心体常有舒缩之力,心房常有启闭之机,若用药一于补敛,实恐于舒缩启闭之运动有所妨碍,故又少加乳香没药之流通气血者以调和之。其心中兼热用生地者,因生地既能生血以补虚,尤善凉血而清热,故又宜视热之轻重而斟酌加之也。
It is said in the book
Nei Jingthat, the heart is the place where the Heart-spirit lives. The spirit chooses the heart as its living place, so it chooses the Qi-blood in the heart as it protect. Whenever the Heart has not sufficient Qi and blood to protect the spirit, the spirit in the heart would not calm itself, so to cause palpitation. The herb Longyanrou nourishes Heart blood; Zaoren and Boziren nourish the Heart Qi. The Longu goes to the Liver to calm down the Liver-spirit (Spirit-Hong), and Muli goes to the Lung to calm down the Lung-spirit (Spirit-Po). The Spirit-Hong and the Spirit-Po are the assistants to the Heart spirit (Heart-Shen). The use of the Longgu, Muli with Shanyurou, can restrain the scattered Heart Qi, so that the Qi of the three Jiao can also be restrained together. The function of the heart is to pump blood. The body of the heart bears the expanding and contracting nature; and the heart body has also opening and closing function. If use only one herb to nourish or restrain, there is worry that it may affect the expanding-restraining, or the opening-closing function of the heart. So, little Ruxiang and Moyao are added to balance the movement and the flow of the Qi and blood. If the heart feels Hot, the Shengdi is added, since it can nourish the blood, so to correct the insufficient status. It is especially useful to cooling the blood and to clear the Hot, so it is used according to the level of the Hot.   

西人曰:人身心肺关系尤重,与脑相等。凡关系重者、护持之尤谨,故脑则有头额等八骨以保护之,而心肺亦有胸胁诸骨以保护之。心肺体质相连,功用亦相倚赖,心之功用关系全体,心病则全体皆受害,心之重如此。然论其体质,不过赤肉所为,其能力专主舒缩,以行血脉。

有左右上下四房;左上房主接肺经赤血;右上房主接周身回血;左下房主发赤血,营运周身;右下房主接上房回血过肺,更换赤血而回左上房;左上房赤血,落左下房入总脉管,以养全体;右上房回血,落右下房上注于肺,以出炭气而接养气。故人一身之血,皆经过于心肺。心能运血周流一身,无一息之停,实时接入,实时发出,其跳跃即其逼发也,以时辰表验试,一 (即一分钟)跳七十五次,每半时跳四千五百次,一昼夜计跳十万八千次。然平人跳不自觉,若觉心跳即是心经改易常度。心房之内左浓于右,左下房浓于右下房几一倍,盖左房主接发赤血,功用尤劳,故亦加浓也。心位在胸中居左,当肋骨第四至第七节,尖当肋骨第五第六之间,下于乳头约一寸至半寸,横向胸骨。病则自觉周遭皆跳,凡心经本体之病,或因心房变薄变浓,或心房之门有病,或夹膜有病,或总管有病。亦如眼目之病,或在明角罩,或在瞳人,或在睛珠,非必处处皆病也。大概心病左多于右因左房功用尤劳故耳。心病约有数端:一者,心体变大,有时略大,或大过一半。因心房之户,有病拦阻,血出入不便,心舒缩之劳过常度。劳多则变大,亦与手足过劳则肿大之理相同。大甚,则逼血舒缩之用因之不灵矣。一者,心房门户变小、或变大、或变窄、或变阔,俱为非宜。盖心血自上房落下房之门,开张容纳血入后,门即翕闭,不令血得回旋上出。其自下房入总管处亦有门,血至则开张使之上出,血出后门即翕闭,不令血得下返。若此处太窄太小,则血不易出。太大太阔,则血逼发不尽,或已出复返,营运不如常度矣。

再者心跳:凡无病之人心跳每不自觉。若因病而跳时时自觉,抚之或觉动。然此证有真有假:真者心自病而跳也,或心未必有病,但因身虚而致心跳,亦以真论;若偶然心跳,其人惊惧,防有心病,其实心本无病,即心跳亦临时之事,是为假心跳证,医者均须细辨。凡心匀跳无止息,侧身而卧,可左可右,呼吸如常,大概心自不病。所虑跳跃不定,或三四次一停,停后复跳不能睡卧,左半身着床愈觉不安,当虑其门户有病,血不回运如常。有停滞妄流而为膨胀者,有累肺而咳嗽、难呼吸或喘者,有累脑而昏蒙头疼,中风慌怯者,有累肝而血聚积满溢者,有累胃不易消化,食后不安,心更跳者,皆心病之关系也。若胸胁骨之下有时动悸,人或疑为心跳,其实因胃不消化、内有风气,与心跳病无涉,虚弱人及妇女患者最多,略服补胃及微利药可也。

按:西人论心跳证有真假,真者手扪之实觉其跳,假者手扪之不觉其跳。其真跳者又分两种:一为心体自病,若心房门户变大小窄阔之类,可用定心汤,将方中乳香没药皆改用三钱,更加当归丹参各三钱;一为心自不病,因身弱而累心致跳,当用治劳瘵诸方治之。至假心跳即怔忡证也,其收发血脉之动力,非大于常率,故以手扪之不觉其跳。特因气血虚而神明亦虚,即心之寻常舒缩,徐徐跳动,神明当之,亦若有冲激之势,多生惊恐,此等证治以定心汤时,磨取铁锈水煎药更佳。至于用铁锈之说,不但如西人之说,取其能补血分,实借其镇重之力以安心神也。载有一味铁养汤,细观方后治验诸案,自知铁锈之妙用。惟怔忡由于大气下陷者,断不宜用。

Comments: the western medicine also talks about the true palpitation and false palpitation. In the true palpitation, with hand to touch the heart, it can feel the beat of the heart. In false palpitation, the hand cannot feel the beat when the hand touches the heart. For the true palpitation, if the heart is sick, and if the valves of the heart becomes bigger or smaller, the herbal formula Dingxin Tang can be used, among which, the Ruxiang and Moyao can be changed to 9 gram each, with addition of Danggui and Danshen 9 gram each. Another kind of the true palpitation is: there is not disease in the heart, but the body is weak, that influence the heart to cause palpitation. In this condition, use various herbal formulas for Laoji (exhausting condition). For the false palpitation, the pumping power of the heart is not bigger than the normal condition, so the heart beat can not be touched with hand. It is due to the deficient conditions that cause the deficiency in the Spirits. It means that, the normal heart beat is controlled by the Heart Spirit. If it is stimulated or disturbed, it is scared to cause scaring feeling. When Dingxin Tang is used for such false palpitation, another idea is to also use cutlering water, or rust water, to cook the herbs. The rust water, not only as the western medicine said can nourish the blood, but also can calm the spirit, for its heavy suppressing nature. There is even a formula using the rust water alone as ingredient. Look at the explanation for the formula later, the readers would understand its usefulness. Only if the palpitation is caused by Qi-sink, the rust water should be not used.

 

2.安魂汤

2. Anhong Tang (here, an means calm in Chinese; Hong: Liver spirit).

治心中气血虚损,兼心下停有痰饮,致惊悸不眠。
It works for the condition, in which the Heart Qi and the Heart Blood are deficient, with additional accumulation of Phlegm-Water in the upper abdomen, so to cause scare, palpitation and poor sleep.

龙眼肉(六钱) 酸枣仁(四钱,炒捣) 龙骨(五钱,捣末) 牡蛎(五钱,捣末) 半夏(三钱) 茯苓片(三钱) 生赭石(四钱,轧细)

Longyanrou 18 gram, Suanzaoren 12 gram (fry and grind), Longgu 15 gram (grind), Muli 15 gram (grind), Banxia 9 gram, Fuling 9 gram, Daizheshi 12 gram (grind).

若服一两剂后无效者,可于服汤药之外,临睡时用开水送服西药臭剥一瓦,借其麻痹神经之力,以收一时之效,俾汤剂易于为力也。
If the condition is not improved after one or two doses, a tablet of western medicine Xiubo can be taken before going to bed, together with the the herbal tea. This is to take its advantage of anesthesia function for a short time help, so to have longer time to allow the herbs to work.

方书谓:痰饮停于心下,其人多惊悸不寐。盖心,火也,痰饮,水也,火畏水刑,故惊悸至于不寐也。然痰饮停滞于心下者,多由思虑过度,其人心脏气血,恒因思虑而有所伤损。故方中用龙眼肉以补心血,酸枣仁以敛心气,龙骨牡蛎以安魂魄,半夏茯苓以清痰饮,赭石以导引心阳下潜,使之归藏于阴,以成瞌睡之功也。

It is said in the medical book that if there is Phlegm-Water accumulation in the upper abdomen, the person tends to have scare, palpitation and poor sleep. The Heart belongs to Fire; the Phlegm, to the Water. The Fire fears the Water’s attack, so there is scare, palpitation, as to hard to fall into sleep. The reason for the Phlegm however is usually due to over thinking, and over mind work. The Qi and Blood of the Heart is damaged by the over mind work. So, in the formula, the Longyanrou is used to nourish the Heart Qi and blood; Suanzaoren is to restrain the Heart Qi; Longgu and Muli work to calm the Liver-spirit and the Lung-spirit. The Banxia and Fuling are to cleanse the Phelgm-Water; the the Daizheshi direct the Heart Yang Qi down to store into the Yin, so to improve the sleep.

 一媪,年五十余,累月不能眠,屡次服药无效。诊其脉有滑象,且其身形甚丰腴。知其心下停痰也。为制此汤,服两剂而愈。
An old lady, 50 years of old, had hard to fall into sleep. She tried many therapies but all failed. Her pulse was slippery; her body was pretty fat. It is considered that she has Phlegm accumulation in the upper part of the abdomen. She was give this formula. After two doses, her problem was completely solved.

一妇人,年三十许,一月之间未睡片时,自言倦极仿佛欲睡,即无端惊恐而醒。诊其脉左右皆有滑象,遂用苦瓜蒂十枚,焙焦轧细,空心时开水送服,吐出胶痰数碗,觉心中异常舒畅,于临眠之先又送服熟枣仁细末二钱,其夜遂能安睡。后又调以利痰养心安神之药,连服十余剂,其证永不反复矣。
A lady, 30 years of old, had no sleep for almost one month. She complained that she felt very tired and wanted to fall into a sleep, but felt awake by sudden scare and fear. Her pulse was slippery on both sides. So, the formula used to her was: use bitter melon base ten; bake it to charcoal; grind into powder. Drink it in empty stomach with hot water for swallow. After drink of the herbal tea, she spitted out stick phlegm several bowels and felt much easy in the heart. Before she went to bed, she was asked to drink cooked Suanzaoren powder 6 gram. The night, she had very good sleep. Later, she was given Phlegm-cleansing, Heart-nourishing and Spirit-calming herbs. After ten more doses, her problem was completely solved.

《内经》邪客篇有治目不得瞑方。用流水千里以外者八升,扬之万遍,取其清五升煮之,炊以苇薪。水沸,置秫米一升,制半夏(制好之半夏)五合,徐炊令竭为一升半。去其渣饮汁一小杯,日三稍益,以知为度(知觉好也)。故其病新发者,复杯则卧,汗出而已矣,久则三饮而已也。

In the chapter of Xieke, in book Nei Jing, there is a formula to solve poor sleep: use flowing water, thousands of mile away, about 8 liters, raising it with spoon and pouring the water in the spoon down. Repeat this way for tens of thousands times. Collect five liters to bring to boil, with reed burning to bring the water to boil. After boiling, add husked sorghum one liter, processed Banxia125 gram. Cook it until the total volume down to one and half liter. Remove its residue, collect one small cup to drink, three times a day, until the patient felt slight dizzy (the healing mark). For new cases, one cup of the herbal tea can solve the poor sleep; for patient with chronic poor sleeping condition, three cups are usually enough.

观此方之义,其用半夏,并非为其利痰,诚以半夏生当夏半,乃阴阳交换之时,实为由阳入阴之候,故能通阴阳和表里,使心中之阳渐渐潜藏于阴,而入睡乡也。秫米即芦稷之米(俗名高粱),取其汁浆稠润甘缓,以调和半夏之辛烈也。水用长流水,更扬之万遍,名曰:劳水,取其甘缓能滋养也。薪用苇薪,取其能畅发肾气上升,以接引心气下降,而交其阴阳也。观古人每处一方,并其所用之薪与水及其煎法、服法,莫不详悉备载,何其用心之周至哉!
The meaning of this formula: the use of Banxia is not to take its nature to clear phlegm, but to take its conduction of the Yang into the Yin. The herb Banxia is born in the middle of the summer, which is the time when the Yang-Yin exchange, e.g. it can conduct the Yang into the Yin, to make the Heart Yang gradually dive into the Yin. The Yin is the sleeping status.
husked sorghum is the Chinese sorghum. Its juice is stick, sweat and mild in herbal nature. It is used to balance the spicy and vigorous nature of the Banxia. The water used is long-flowing water, which has been lifted for many times, so is termed “Laoshui” (e.g. the exhausting water). Such water is sweat and calm and has nourishing property. The fire is created by burning of reed, for its ability to raise Kidney Qi up and to conduct the Heart Qi down, so to conduct the Yin and Yang. When we see the formula from old Chinese doctors, the herbal ingredients, the water and the way to create the fire, the way to drink the herbal tea or paste, all are very in details. How care they are!

按:《内经》之方多奇验,半夏秫米汤,取半夏能通阴阳,秫米能和脾胃,阴阳通、脾胃和,其人即可安睡。故《内经》谓饮药后,复杯即瞑,言其效之神速也。乃后世因其药简单平常,鲜有用者,则良方竟埋没矣。门生高××治天津刘姓,年四十二,四月未尝少睡,服药无效,问治法于愚,告以半夏秫米汤方。高××因其心下发闷,遂变通经方,先用鲜莱菔四两切丝,煎汤两茶杯,再用其汤煎清半夏四钱服之。时当晚八点钟,其人当夜即能安睡,连服数剂,心下之满闷亦愈。

Comments: the formula from the book Nei Jingusually works very well. …… the later doctors though that it is too simple to use. So very little doctor used it. My student Mr. Gao treated a patient, Mr. Liu in Tianjin city. The patient, 42 years of old, had no good sleep for four months. No any herbal formula had worked for him. Mr. Gao asked me for help. I told him this Banxia Shumi Tang. For the patient had fullness in the upper abdomen, he modified this formula: fresh radish, cut into silk, cooked for two cups of liquid. Use this liquid to cook Banxia 12 gram. On the 8 o’clock that night, the patient could fall into sleep. After several doses, the fullness in the upper abdomen also disappeared.
