1. Comment on the needling handlings (by Dr. Shi Rong, China)

It is common that the clinic healing effects are quite different among acupuncturists, though they use the same needle and perform the same needle on the same acupuncture points. Why? One of the major reason is that the needle handling technique is different.

There are more than 50 kinds of needle handling or manipulating technique in practice. No matter what kind of needle handling technique you use, the most important and the end point is if patient has needle feeling. The needle feeling does not mean the feeling of pain but kind of sour, numb, bloating, cold, or hot feeling. Sometimes it could be a feeling of water wave, ant moving, electric shock, or pulse, or the combinations.

For patient in coma, or deafness, he/she cannot tell the needle feeling. This time, the practitioner should feel the needle feeling as sedimentation, falling down, tight, or non-smooth under the needle. If there is no needle feeling, the practitioner will feel the needle as in an empty space. 

2. Comment on the needling handlings (by Dr. Bao Zhian, China)

Some experience on the needle feeling: under normal condition and generally, it is not so hard to stimulate out the needle feelings to the patient. If there is no any feeling and the practitioner has made sure the location of the needle is correct, then he can leave the needle for sometime to “wait” for the needle feeling. It only needs about 10 min to wait. Another way to cause the needle feeling is to handle the needle. You can change the direction of the needle, or the angle of the needle in the skin. You can also twist the needle little bit, insert deeper and pulling up little bit, scratch the needle up or down, shacking the needle in various ways, insert two or three needles into the same point. All help to induce the needle feeling.

To further improve the healing effect, we need to control the direction the strength of the needle feeling too. One of the ways is to insert a needle in the far end of the meridian to direct the needle feeling.

To perform acupuncture, we also need to pay attention to the body condition of patient and the time of acupuncture. We need to diagnose the body condition as weakness (deficiency) or excess, hot/fire or cold. For stronger person, the stimulation (the needle feeling) can be induced stronger, but for weak person (long time illness, or elderly, or after heavy disease/surgery patients), the stimulation should be little. For some kind of disease, the proper time for acupuncture is also variable. For insomnia (poor sleep), acupuncture is best performed half hour before bed. For nervous pain, it is best performed right during the pain.

3. Acupuncture with moving of body (by Dr. Shi Rong, China)

There are lots of techniques to handle the needle during acupuncture. Most of the techniques pay attention how to handle the needle in the spot, such as how to insert, to wait, to keep, to induce feelings, etc. The acupuncture with moving body pays attention how to stimulate whole body reaction by moving the arms or legs. It is useful for the treatment of pain due to muscles, nerves on the surface of the body. It asks to insert the needle into the acupuncture point while moving the affected arm or leg. First, move the arm or leg little bit by the practitioner, then by the patient himself. (Martin: also, move the non-affected arm or leg first, then the affected arm or leg).

Clinic experience tells us: the pain-stop effect by acupuncture might be short time without moving the extremes of the body. With the body-moving technique, it lasts longer time.

4. Acupuncture on lower part of the body to treat illness on the upper part of the body (by Dr. Li Baozhong, China)

I have a patient (family name is He), 58 years of old. His complain is ringing in the brain (not in the ear) for 5 years. He has tried various drugs from western medicine and herbs and acupuncture somewhere for a long time, without clear improvement. The problem becomes worse year after year and now starts to have dizziness and ringing in the ear too, so that he feels hard to fall into sleep. When he comes to me, I chose acupuncture Guanyuan (on lower stomach), Qihai, (on lower stomach), Zusanli (on leg) and Taixi (on foot). One treatment largely improved the symptoms. It is completely fine after five days treatment.

This is an example that we do not necessarily need to use needle on the head when the illness is in the brain.  Based on the condition of the whole body (the patient is in a weak and Qi deficiency condition), adjusting the whole body condition can also solve the problem, and even faster.

5. Tiantu point for loss of voice  (by Dr. Dong Guilan, China)

Loss of voice (aphasia) in hysteria is commonly due to extreme angry, scared, fear, etc. We use the acupuncture point of Tiantu for the treatment.

Let patient sit down, with head little bit back bend. Fast insert the acupuncture needle into the Tiantu point (on the throat), deeper along the back of the thoracic bone about 1 cm. Twist the needle to stimulate the needle feeling. Leave the needle for 5 min.

6. Press Geguan point for stomach pain (by Dr. Li Yiqing, China)

Many times, the healing of an illness can also be achieved by press acupuncture point. Here is example: press Geguan point (on the back, under the seventh thoracic spinal, then 4 cm to the left or right side: two points). Normally we only use the right point to press. It is very good to stop acute pain in the stomach. The acuter the more effective. When practice, let the patient sit down on chair (or lie with face down). Press the right Geguan point with your finger. Press it with little force, then increase the force gradually until the patient cannot tolerate. Do not change the force light or heavy. During the treatment, if the patient has saliva in mouth, let him swallow it. More saliva suggests better results. Press for about 15 min. Usually it works well. If not, then press the left Geguan point the same way.

7. Press Zhongshu point for stomach pain (by Dr. Li Yiqing, China)

Zhongshu point is under the tenth thoracic spinal, on the back middle line of spine. Press this point is good for the treatment of stomach pain due to roundworm.

This is goodness of the Traditional Chinese Medicine: if we have no herbs, we can use acupuncture. If we have no needles to perform acupuncture, we can use our fingers!

8. Acupuncture on Jinsuo point for stomach pain (by Dr. Li Yiqing, China)

Jinsuo point, as the Chinese name indicates, means the spasm of muscle/tendons. Acupuncture on this point is find really helpful in the treatment of acute stomach pain due to the spasm of  musculus rectus abdominis. (Dr. Martin: actually you should not forget to use this point whenever you meet a patient with spasm on calf, or migraine headache). Jinsuo point is located under the 7th thoracic spinal.

9. Acupuncture for acute diarrhea (by Dr. Yang Yuande, China)

With acute diarrhea, patient feels fever, irritable within anus, urgent feeling to have bowel movement again and again, dry mouth.

We normally use the following two groups of acupuncture points for the treatment:

(1). Qianshu, Shangjuxu, Hegu, Quchi, Neiting.

(2). Daheng, Yinlingquan, Diji, Sanyinjiao.

Both with depletion technique (Even technique does not work well). Leave needle for 30 to 120 min with needle stimulation every 10 min. For severe cases, repeat acupuncture 2 to 3 times a day.

Acupuncture is very effective in the treatment of acute bacterial diarrhea.

10. Acupuncture on pain point to stop pain (by Dr. Shi Rong, China)

The pain point here means the point feels pain when press, so we call it reaction point. It does not mean the actual pain point that patient feels. For example, patient feels pain in the teeth and come to us for stop the pain on teeth. But we can find some points in the body that make the patient feels pain only when press. Without press on this point, patient never realizes that there is pain there. 

(Dr. Martin: there are several ways to find the reaction points: (1). Press the surrounding area around the pain point/area); (2). Press along the corresponding meridian. For example, press the stomach meridian if patient has pain in stomach; or press lung meridian if patient has pain in chest due to cough; (3). Press the sister meridian. For example above, if patient has pain in stomach, we can also press Spleen meridian since the Stomach meridian and the Spleen meridian is sister. (4). Press the back part of Urine bladder meridian, since in this part of the meridian, there are point links to the inner organs. For example, there is a point called Stomach Shu on the Urine Bladder meridian, that is linked to the stomach in the belly. For most people, there could be many reaction points. Choose the most painful one for acupuncture.

11. Acupuncture for bedwetting (by Dr. Zhao Guoxin, China)

Normally, child over 3 years of old will stop bedwetting. If not, there should be something done for them to stop the wetting.

Acupuncture points commonly used are Guanyuan (supplying), Shenshu (supplying) and Sanyinjiao, with the additional use of Pangguanshu and Zhongji point. It works for most children (or for adult too). Sometimes there are difficult cases, especially for girls. In this case, we choose the point of Tongli (on the inside of wrist) and Dazhong (on the inside of ankle). This acupuncture point couple works to connect the Heart and Kidney meridian (system). Generally it needs about 5 to 7 times of treatment.

12. Moxa therapy for bedwetting (by Dr. Zheng Yizhong, China)

Moxa works to promote the transportation of blood and Qi, as well as life energy moving in the body. Its effectiveness cannot be replaced by acupuncture therapy. Especially for patients who is very week, and who fears needle, or who didn’t have clearly improvement with the acupuncture treatment, moxa can be one of the best alternatives. I have been using moxa for the treatment of bedwetting in children with pretty good result. Normally it needs moxa once a day, for 20 to 30 days. The point for moxa is Guanyuan.

13. Acupuncture on Renzhong (Shuigou) point for acute lower back pain (by Dr. Guo Shigui, China)

Renzhong point is located on the Dou meridian. It can be used for the treatment of acute lower back pain. This is an example of choosing upper acupuncture points for the treatment of illness in the lower part of body. When acupuncture on this point, slightly twist the needle until the patient feels tear. At the same time, let patient moves body little bit.

14 Acupuncture for herpes zoster (by Dr. Guo Shigui, China)

Herpes zoster is a kind of virus-caused skin disease. It shows blisters on the skin with terrible pain. It can last for 2 to 4 weeks and without very effective therapy both western medicine or Traditional Chinese Medicine. Here we introduce a way with the application of acupuncture therapy.

Bound several (7 to 8) needles together. Use it to break the blisters, then use suction cupping therapy to suck off the liquid in the blister. Finally apply Zijingding paste on the lesion. This way works pretty well.

15. Acupuncture for herpes zoster (by Dr. Tian Suqin, China)

Here introduces another way for the treatment of herpes zoster. Insert needle from the normal skin on the one side of the herpes, along the long axis of the skin lesion,  insert the needle parallel to the long axis of lesion, to the another end of normal skin. Leave needle for 20 min. For bigger lesion, use needle around the lesion.

16. Acupuncture for pregnancy vomiting (by Dr. Ji Qingshan, China

Vomiting during the early stage of pregnancy is hard to treat with herbal therapy since patient can hardly swallow anything. It however works well with acupuncture. The commonly used points in our clinic is Zhongwan, Tianshu, Neiguan and Zusanli. All are with supplying technique (nourishing technique).

17. Acupuncture for delivery of dead fetus (by Dr. Gao Yu, China)

When a fetus is dead in the uterus and the pregnancy woman has to energy to deliver it out, the only choice with western medicine is surgery. With acupuncture, the woman may not need to suffer from the surgery so no possible side effect with the surgical therapy.

Acupuncture on Hegu (both side, with supply technique), then Sanyinjiao (both side, depletion technique), then Chuichan point (it is located three body inches beside Guanyuan on the lower stomach. Use supplying technique and twist the needle on both sides the same time).
